QIC-AG Partner Site Surveys

Post Permanency surveys are a central element to the work with partner sites to better understand what families need post adoption or post guardianship.  Below are materials including the survey used in Vermont and adaptations of that survey used in North Dakota and Wisconsin.

Within the Universal Interval of the Permanency Continuum, Vermont implemented a Permanency Survey.  Below you will find the surveys used.
